Every day in clinical practice, patients arrive at appointments carrying a mental list of their prescription medications—carefully rehearsed, often written down. What frequently goes unmentioned, however, is the collection of supplements sitting on the kitchen counter: the fish oil capsules taken for heart health, the St. John's Wort purchased at the pharmacy for low mood, the ginkgo biloba recommended by a neighbor for memory. These products feel different from medications. They feel natural, benign, and entirely separate from the conversation.
They are not.
Dietary supplements, herbal preparations, and high-dose vitamins are pharmacologically active substances. They interact with enzymes, alter absorption pathways, and can meaningfully change how prescription drugs behave in the human body. According to research published in peer-reviewed medical literature, a significant proportion of Americans who take prescription medications also use dietary supplements—and the majority do not disclose this to their healthcare providers. This communication gap is not a minor oversight. In some cases, it is genuinely dangerous.
Why Patients Don't Mention Supplements
The reasons patients omit supplements from their medication histories are understandable, even if the consequences are not. Many individuals simply do not consider supplements to be "real" medicine. Others worry about being judged for pursuing complementary approaches. Some assume that because a product is sold without a prescription, it carries no clinical significance.
There is also the matter of the appointment itself. Time is limited. Patients prioritize the symptoms they came to discuss, and the supplements seem beside the point.

The Most Clinically Significant Interactions
St. John's Wort and Nearly Everything
St. John's Wort (Hypericum perforatum) is one of the most widely used herbal supplements in the United States, taken primarily for mild depressive symptoms. It is also one of the most pharmacologically disruptive. This herb is a potent inducer of cytochrome P450 enzymes and P-glycoprotein—two systems the body uses to metabolize and transport a broad range of medications.
When St. John's Wort accelerates these pathways, it causes the body to clear certain drugs too quickly, reducing their effectiveness to potentially dangerous levels. Affected medications include antiretrovirals used to manage HIV, cyclosporine (an immunosuppressant critical for transplant patients), warfarin (a blood thinner), oral contraceptives, and several antidepressants. There are documented cases of organ rejection in transplant recipients and unintended pregnancies linked to this interaction. The risk is not theoretical.
Additionally, when taken alongside serotonergic medications—including SSRIs and SNRIs—St. John's Wort can contribute to serotonin syndrome, a potentially life-threatening condition characterized by agitation, rapid heart rate, high blood pressure, and in severe cases, seizures.
Ginkgo Biloba and Bleeding Risk
Ginkgo biloba is frequently marketed for cognitive support and memory enhancement. It has antiplatelet properties, meaning it inhibits the clumping of platelets that is essential to normal blood clotting. On its own, this effect is modest. Combined with prescription anticoagulants such as warfarin, clopidogrel, or aspirin therapy, however, ginkgo can significantly amplify bleeding risk.
This is particularly relevant for older adults, who are more likely to be taking blood-thinning medications and more likely to experience serious complications from abnormal bleeding—including gastrointestinal hemorrhage and intracranial bleeding.
Fish Oil and Blood Thinners
High-dose omega-3 fatty acid supplementation—typically defined as more than three grams per day—has measurable antiplatelet and anticoagulant effects. For patients already prescribed blood thinners, the additive effect can push clotting parameters outside the therapeutic range. This is not a reason to avoid fish oil categorically; at the doses found in most standard supplements, the interaction risk is generally low. However, patients taking prescription anticoagulants should always disclose their omega-3 use so that their physician can monitor accordingly.
Vitamin K and Warfarin
Warfarin works by interfering with vitamin K-dependent clotting factors. Any supplement that contains significant amounts of vitamin K—including certain greens-based powders, multivitamins, and standalone vitamin K supplements—can directly counteract the drug's intended effect. The interaction is well-established, and patients on warfarin typically receive dietary counseling around vitamin K. What is less commonly addressed is the vitamin K content of supplements, which can vary considerably by product and formulation.
Calcium, Iron, and Antibiotic Absorption
Some interactions are not about toxicity but about efficacy. Calcium and iron supplements bind to certain antibiotics—particularly fluoroquinolones and tetracyclines—in the gastrointestinal tract, forming insoluble complexes that the body cannot absorb properly. The result is a significantly reduced antibiotic blood level, potentially leaving an infection undertreated. Timing the supplement and the antibiotic appropriately can mitigate this, but only if the prescribing physician knows the supplement is being taken.
Preparing for a Transparent Conversation
