Inflammaging: The Hidden Fire Inside Your Body That's Stealing Your Years

Most people associate inflammation with something visible — a swollen ankle, a red insect bite, the ache of a sore throat. These are acute inflammatory responses: purposeful, temporary, and ultimately healing. But there is another kind of inflammation that operates entirely beneath your awareness, producing no dramatic symptoms while quietly accelerating the biological processes that age your cells, stiffen your arteries, and increase your risk for conditions ranging from type 2 diabetes to Alzheimer's disease.

Scientists have given this phenomenon a name: inflammaging — a portmanteau of inflammation and aging that has moved from academic journals into mainstream clinical medicine over the past decade. As a physician focused on preventive care, I find this area of research both compelling and actionable. The good news is that chronic low-grade inflammation is not invisible to modern medicine. We can measure it, and in many cases, we can meaningfully reduce it.

What Is Chronic Low-Grade Inflammation?

Your immune system is designed to respond to threats — pathogens, injuries, toxins — with a precisely calibrated inflammatory cascade. Once the threat resolves, the system is supposed to stand down. Chronic low-grade inflammation occurs when that stand-down signal never fully arrives. The immune system remains in a state of low-level activation, releasing inflammatory mediators — cytokines, chemokines, and reactive oxygen species — on a continuous basis.

Unlike the acute inflammation that follows a sprained wrist, this process is systemic. It circulates throughout the body, interacting with virtually every organ system. Over years and decades, this persistent biochemical environment damages DNA, shortens telomeres (the protective caps on chromosomes that serve as a biological clock), accelerates arterial plaque formation, disrupts insulin signaling, and promotes the cellular dysfunction associated with neurodegenerative disease.

The drivers are numerous and, importantly, largely modifiable: excess body fat (particularly visceral adipose tissue), poor dietary patterns, inadequate sleep, chronic psychological stress, sedentary behavior, gut microbiome imbalances, and environmental exposures including tobacco smoke and air pollution.

Biomarkers Worth Discussing With Your Doctor

One of the most empowering conversations I have with patients is explaining that we do not have to guess at their inflammatory status. Several validated laboratory tests can provide meaningful clinical insight. If you are concerned about chronic inflammation, consider asking your physician about the following:

High-Sensitivity C-Reactive Protein (hs-CRP) This is the most widely used clinical marker of systemic inflammation. Produced by the liver in response to inflammatory signaling, hs-CRP levels below 1.0 mg/L are generally considered low risk; levels between 1.0 and 3.0 mg/L indicate moderate risk; and levels above 3.0 mg/L (in the absence of acute illness or injury) suggest elevated chronic inflammation. This is a standard blood test covered by most insurance plans when ordered with appropriate clinical justification.

Erythrocyte Sedimentation Rate (ESR) A less specific but complementary marker, ESR measures how quickly red blood cells settle in a test tube — a process accelerated by inflammatory proteins in the blood. It is particularly useful when assessed alongside hs-CRP.

Fasting Insulin and Hemoglobin A1c Insulin resistance is both a consequence and a driver of chronic inflammation. Elevated fasting insulin — even in individuals with normal fasting glucose — can signal early metabolic dysfunction and inflammatory burden.

Homocysteine Elevated homocysteine levels are associated with vascular inflammation and increased cardiovascular risk. This marker is especially relevant for patients with a family history of heart disease or stroke.

Omega-3 Index While not yet universally standard, this test measures the percentage of EPA and DHA fatty acids in red blood cell membranes and serves as a proxy for dietary anti-inflammatory fatty acid status. An index below 4% is associated with significantly higher inflammatory risk.

I want to be clear: no single biomarker tells the complete story. These values should always be interpreted within the full context of your health history, physical examination, and lifestyle factors. The goal is not to chase numbers in isolation but to build a comprehensive clinical picture.

Evidence-Based Strategies to Reduce Inflammation

The following recommendations reflect both the current scientific literature and the practical guidance I offer patients in my own practice. None of these require a prescription — though working with your physician to personalize and monitor your approach is always advisable.

Prioritize an Anti-Inflammatory Dietary Pattern

The Mediterranean dietary pattern remains the most extensively studied and consistently supported nutritional framework for reducing inflammatory markers. This means emphasizing vegetables, legumes, whole grains, fatty fish (salmon, mackerel, sardines), olive oil, nuts, and fruit — while limiting ultra-processed foods, refined carbohydrates, and red and processed meats. Specific foods with robust anti-inflammatory evidence include turmeric (curcumin), dark leafy greens, berries rich in anthocyanins, and walnuts.

For patients with significant visceral adiposity, even modest weight reduction — 5 to 10 percent of body weight — has been shown to produce measurable decreases in hs-CRP.

Treat Sleep as a Clinical Priority

Sleep deprivation is one of the most potent and underappreciated inflammatory triggers. Research consistently demonstrates that sleeping fewer than six hours per night is associated with elevated interleukin-6 and tumor necrosis factor-alpha — two key pro-inflammatory cytokines. Seven to nine hours of quality sleep per night is not a lifestyle luxury; it is a physiological necessity. If you struggle with sleep quality, please raise this with your physician, as conditions such as obstructive sleep apnea are both common and highly treatable.

Manage Psychological Stress Deliberately

Chronic psychological stress activates the hypothalamic-pituitary-adrenal axis, sustaining cortisol release and suppressing immune regulation in ways that directly promote systemic inflammation. Evidence-based stress reduction strategies — including mindfulness-based stress reduction (MBSR), regular aerobic exercise, and behavioral therapy — have demonstrated measurable reductions in inflammatory markers in clinical trials. This is not a peripheral concern; it is central to your biology.

Move Your Body Consistently

Moderate, regular physical activity — 150 minutes per week of moderate-intensity aerobic exercise, as recommended by the American Heart Association — has well-documented anti-inflammatory effects. Importantly, the operative word is moderate. Extreme, prolonged exercise without adequate recovery can paradoxically elevate inflammatory markers. Consistency over intensity is the clinical message here.

A Final Word From My Practice

Inflammaging is not inevitable. The biological processes that accelerate aging in the presence of chronic inflammation are, to a meaningful degree, responsive to the choices we make every day. As a physician, my role is to help you understand your individual risk, interpret your biomarkers in context, and build a personalized, sustainable plan — not to prescribe a generic wellness checklist.
